It looks like it's the real deal. The part number comes up on the PT spec manual at the bottom of page 40 http://www.hnsa.org/doc/pt/specs/part2.htm#pg39:

I DID IT AGAIN! CONSIDER THIS YOUR OPERATIONAL READINESS EXERCISE(ORE)! You guys once again passed with flying colors! This was just an exercise to allow you to all, once again, to come together as a TEAM and hone your skills as top notch PT BOAT researchers! BRAVO ZULU TO ALL!

The 565-622 were the last boats produced by Elco but I don't know why the changes.

Another unique change near the running light was the 565-622 (according to a drawing) also had an air horn mounted to the running light screen:
